    During your appointments, we can explore practical tools and strategies to help daily activities feel more manageable and support your confidence and to remain as independent as possible

Community living support

  • Community living support to help you reconnect with hobbies, routines and community activities that are meaningful to you.

    This may involve identifying any barriers, such as transport, confidence or fatigue, and plan manageable steps so you can build confidence and take part in the activities you value at a pace that feels right for you
  • Emotional wellbeing support to help you adjust to changes in your health, abilities or daily life.

    We can help you explore coping strategies, build confidence, recognise what helps you feel more in control, and signposting you to further support or resources where needed.
  • Falls prevention advice to make you feel safer and more confident at home and in your daily routines.

    Support can include identifying what may increase your risk of falling, such as difficulties moving around, environment or getting on and off furniture, then suggest practical changes, equipment or support to help reduce falls risks and maintain your independence and confidence
Practical and Personalised Support
  • Fatigue management to help you make the most of your energy and feel more in control of your day.

    We can work with you to plan and pace daily activities, build in rest breaks, adapt tasks where needed, and find a balance between the daily tasks you need to do and the activities that matter most to you
  • Guidance for family, carers and your support networks helping the people around you feel informed and confident in supporting you.

    We can offer practical advice on ways they can encourage your independence, communicate in a way that works well for you, and provide safe, appropriate support that respects your choices, routines and goals
  • Home safety advice and recommendations for equipment/adaptations making your home feel safer, more accessible and better suited to your needs.

    During your appointments, we can identify any areas of concern and suggest practical changes, such as equipment, adaptations or layout changes, to support your safety, confidence and independence

Free Initial Consultation

  • Practical ways to manage daily routines and activities to help everyday tasks feel safer, easier and more manageable.

    Together, we can identify the activities you are finding difficult, then explore practical changes, equipment or adjustments that support your independence, confidence and safety
  • Self-care techniques to support your wellbeing and daily self-care.

    We can explore routines, strategies and small changes that help you make looking after yourself a regular and meaningful priority. This may include ways to manage stress, balance the activities you value with ‘true’ rest, build healthy habits, recognise when extra support may be helpful, and feel more confident managing your wellbeing
  • Self-management approaches to help you understand how your condition affects your daily life and build confidence in managing your symptoms.

    This support can help you recognise early warning signs, notice what helps, build routines that suit you, and use practical strategies that support condition and/or symptom management, independence and day-to-day confidence
  • Sleep improvement strategies to help you develop a more restful routine and support your energy, wellbeing and day-to-day life.

    Your appointments may include exploring your sleep patterns, daily routines, fatigue and sleeping environment, then suggest practical strategies that feel achievable for you and support better sleep

Vocational rehabilitation and work-related support

  • Vocational rehabilitation and work-related support including advice on returning to work and staying in work.We offer workplace assessments at a date and time suitable for you and your employer.

    Together, we can explore your job role, work demands, routines and any challenges you may be experiencing, then recommend reasonable adjustments, workplace equipment or practical strategies to support your confidence, wellbeing and ability to return to work or stay well at work
